The Relation between Volume and Outcome of Transcatheter and Surgical Aortic Valve Replacement: A Systematic Review and Meta-Analysis

Abstract: Transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR) are standard procedures for dealing with severe aortic stenosis patients. Researchers have not carried out a systematic review of the volume-outcome relationship in TAVR and SAVR. Our study is intended to address this problem. We systemically searched databases through MEDLINE, EMBASE, PUBMED, and the Cochrane Library up to September 2019. Two reviewers independently screened for the studies and evaluated bias. We used s… Show more

“…First, we cannot rule out residual confounding as the cause. Second, this observation may be indicative of lower operator and institutional experience, which has been found to be associated with patient outcomes 27 28. The volume-outcome hypothesis does not explain why higher TAVR/SAVR ratios have improved outcomes even in SAVR patients.…”

“…There is little information available about the effect of the size or type of hospital on the diagnosis and management of patients with AS. On the one hand, it is well known that high procedural volume translates into better outcomes for both SAVR and TAVI procedure 9–12. On the other hand, there are little data on the centre type and its relation to outcomes.…”

“…In the present study, even a modeled strict enforcement of a volume threshold as a structural quality measure was not projected to improve population-level TAVI outcomes. TAVI site procedural volume has a demonstrable link to outcomes making it a theoretically appropriate structural indicator of quality . However, structural indicators of quality are surrogates in that they measure the capacity to provide high-quality care and do not actually measure the provision or receipt of quality care.…”

“…20 The TVT composite model is a National Quality Forum-endorsed hierarchical proportional odds regression model that was developed for national outcomes benchmarking. 31 The best outcome site in each HRR was defined as the site with the lowest adjusted odds ratio for the composite outcome. Best outcome sites were then grouped and descriptively compared with the remaining sites.…”
