The relative contribution of mechanical stress and systemic processes in different types of osteoarthritis: the NEO study

Abstract: In knee OA, whether or not in co-occurrence with hand OA, surrogates for mechanical stress are suggested to be the most important risk factors, whereas in hand OA alone, surrogates for systemic processes are the most important risk factors.

“…Here, the model of HIV-1 infection further supports the ‘metabolic’ OA phenotype because the prevalence of HOA was greater in patients with MetS than without MetS 17–19. Of note, obesity was not associated with HOA diagnosis in the METAFIB-OA study: such a result agrees with those from the population-based Netherlands Epidemiology of Obesity cohort, finding HOA linked more to metabolic systemic factors than weight itself 22. Radiographic severity was more severe in patients with MetS, so MetS is a risk factor and also an aggravating factor of radiographic HOA during HIV infection.…”

“…Four studies described the adjusted OR [7, 1113]. After pooling the adjusted ORs, the results remained significant (pooled adjusted OR = 1.05, 95% CI = 1.03–1.07, and p < 0.00001) (Figure 5).…”

“…After a systematic review, 8 studies with a total of 3,202 cases and 20,968 controls were finally included. Among these studies, there were two prospective studies [7, 13], three cross-sectional studies [9, 10, 12], and three case-control studies [8, 11]. One study was excluded for lack of data [18].…”

“…Moreover, overweight was found to be associated with hand OA as well, indicating a possible underlying systemic factor in disease pathogenesis 7 . The involvement of both metabolic and systemic aspects was demonstrated in cohort studies, where knee OA development did not depend on weight or BMI but was strongly related to concurrent dysregulation of glucose and lipid metabolism 8 .…”
