The role of adjuvant external beam radiation therapy for papillary thyroid carcinoma invading the trachea

Abstract: PurposeTo evaluate the effect of adjuvant external beam radiation therapy (EBRT) on local failure-free survival rate (LFFS) for papillary thyroid cancer (PTC) invading the trachea.Materials and MethodsFifty-six patients with locally advanced PTC invading the trachea were treated with surgical resection. After surgery, 21 patients received adjuvant EBRT and radioactive iodine therapy (EBRT group) and 35 patients were treated with radioactive iodine therapy (control group).ResultsThe age range was 26–87 years (m… Show more

“…In one study of thyroidectomy, lymph node dissection and shave resection of the trachea with radioactive iodine and selected adjuvant external beam radiation, all patients experienced dysphagia after radiation (125). Hospital mortality ranged from 0.02% to 1.4% in this group.…”

“…Median 5-year survival in patients with thyroid malignancy was 74.2% [range, <10% after shave resection (29) to 100%]. (6)(7)(8)17,36,53,58,73,121,(125)(126)(127)(128)(129)(130)(131)(132)(133)(134)] …”

“…Adjuvant RT has proven effective in reducing LRR in patients who have high-risk factors, such as locally invasive tumors, multiple lymph node )LN( metastases, and microscopic or macroscopic residual disease. [4][5][6][7][8][9] However, only a few studies have reported the survival benefits of ART for recurrent PTC. 10 The difference in survival outcomes between patients with locoregionally recurrent PTC who received ART and those who did not receive ART after salvage surgery has not been wellstudied.…”

“…Such conservative approaches depend on postoperative RAI administration with or without EBRT to manage microscopic disease 3 . These patients may have better local failure-free survival rate (LFFS) with Adjuvant EBRT 6 . Our patient underwent the conservative approach and had positive excision margins.…”
