The safety, effectiveness and cost‐effectiveness of cytisine in achieving six‐month continuous smoking abstinence in tuberculosis patients—protocol for a double‐blind, placebo‐controlled randomized trial

Abstract: Background and aimsTuberculosis (TB) patients who quit smoking have much better disease outcomes than those who continue to smoke. In general populations, behavioural support combined with pharmacotherapy is the most effective strategy in helping people to quit. However, there is no evidence for the effectiveness of this strategy in TB patients who smoke. We will assess the safety, effectiveness and cost‐effectiveness of cytisine—a low‐cost plant‐derived nicotine substitute—for smoking cessation in TB patients… Show more

“…One of the possible less expensive pharmacotherapy options is cytisine, whose efficacy compares to that of currently licensed products. A study from Bangladesh 8 may bring interesting findings; two face-to-face behavioural support sessions will be delivered and cytisine (active arm) or matching placebo (control arm) will be administered in a 25-day course. The success rate may depend on pharmacotherapy duration (at least 3 months) as well as on the type of intervention provided.…”

While medications are widely recommended, their costs prohibit their use in LMICs 8 .…”

“…A cross-sectional survey was conducted among TB patients to assess their eligibility and recruit them to a smoking cessation randomized controlled trial 21 . The trial assessed the effectiveness and cost-effectiveness of cytisine plus behavioural support compared with placebo plus behavioural support.…”

“…We will also record contacts with health services for TB by patients in each trial arm, using the questionnaires adapted from those used in a previous trial of TB [44]. Applying unit costs of health care to quantities recorded in the service use questionnaire will produce a health care cost profile for each patient at baseline, three and six-month follow up.…”
