The scars of time: the disappearance of peptic ulcer-related pyloric stenosis through the 20th century

Abstract: The reduction in pyloric stenosis over the last several decades is disproportionately greater than the change seen in peptic ulcer disease requiring surgery. Our findings suggest that this reduction in pyloric stenosis is largely the result of the changing natural history of the disease rather than due to the introduction of acid-suppressing medication.

“…Pyloric stenosis or GOO, the major cause of metabolic alkalosis is often treated using surgical or endoscopic procedures 3,4) . Although most published cases related to metabolic alkalosis and acute kidney injury were caused by pyloric ulcers, the incidence of peptic ulcer-related disease has decreased steadily worldwide 5,6) . Table 2 revealed few reports related to gastric cancer-induced GOO resulting in metabolic alkalosis and acute kidney injury 7) .…”

“…Gastric adenocarcinoma is important to consider despite its rarity in the western countries likely due to the decreased prevalence of H. pylori . [ 5 6 7 ] Other less common malignancies include gastric lymphoma, neuroendocrine tumors, local extension of biliary malignancies, and duodenal adenocarcinomas. Benign etiologies include PUD, Crohn's disease, pancreatitis, prolapsed gastric polyps, eating disorders,[ 8 ] gastric bezoar, gastric volvulus, Bouveret syndrome,[ 9 ] and superior mesenteric artery syndrome.…”

“…Peptic ulcer-related disease has steadily reduced in incidence on a worldwide basis. Interestingly, this trend started prior to the introduction of acid-suppressing medication suggesting a more fundamental change in the natural course of the disease process 1. Rarer causes are adult-onset idiopathic hypertrophy, gastric neoplasms, gastric diverticulosis and iatrogenic following endoscopic submucosal dissection.…”
