The Socket-Shield Technique: First Histological, Clinical, and Volumetrical Observations after Separation of the Buccal Tooth Segment - A Pilot Study

Abstract: The applied modification seems not to interfere with implant osseointegration and may still preserve the buccal plate. It may offer a feasible treatment option for vertically fractured teeth.

“…A mean loss of 0.66 mm was observed in the labial direction after leaving the root segment for 5 months, and the mean loss of distance was 0.88 mm 6 months later. The loss in the labial direction may be because the provisional restoration was not available to shape the gingival profile (Bäumer et al., ). In our study, the loss of labial bone after 3 months was 0.57 ± 0.46 mm when leaving the buccal root segment and blood clot in the extraction socket and 0.37 ± 0.35 mm when leaving the buccal root segment and Bio‐Oss Collagen in the extraction socket.…”

“…Baumer ve ark. 11 köpekler üzerinde yaptıkları çalışmada 4. ve 5. premolar dişleri hemiseksiyon ile ikiye ayrılıp bukkal kök segmentlerine direk temas edecek şekilde implantlar yerleştirmişler, iyileşme sonunda osseointegrasyonda herhangi bir problem olmadığı ve diş fragmanı ile implant arasında kemik oluştuğu gözlemişlerdir. “…In most situations, these changes adversely affect with the esthetic outcome, treatment planning, implant positioning, material selection, and osseointegration . This is even more critical in the anterior regions where these changes directly influence red and white esthetics . Soft‐tissue augmentations immediate or posterior to implant placement are successful to control the tissue alterations.…”

“…Several approaches have been described for contouring the socket alterations caused by tooth extraction: implant placement directly after extraction; positioning of the implant on the palatal/lingual wall (“palatal approach”), preserving the buccal wall contact; performing the surgery using the flapless technique to maintain vascularization; and using soft‐tissue or bone grafts to maintain the dimension of the ridge by socket augmentation . Recent studies concentrated either on immediate implants or on the use of grafts, but they also stated that remodeling cannot be avoided with these techniques but can continue even after 3 to 6 months of healing .…”
