The value of multimodality imaging in diagnosis and treatment of cardiac lipoma

Abstract: Background Cardiac lipoma is a rare primary tumor in the heart and pericardium. Multimodality imaging methods, especially magnetic resonance imaging (MRI), are crucial in detecting and diagnosing cardiac lipomas. Besides, they are of significant importance in management of cardiac lipomas. The aim of this study was to evaluate the value of multimodality imaging methods in diagnosing and treatment of cardiac lipoma by describing a series of cases of cardiac lipoma. … Show more

“…Lipomas show the same signal intensity as subcutaneous fat in all CMR sequences. They present as homogeneous hyperintense compared with myocardium on T1WI and the characteristic complete signal loss of the mass is seen in the fat suppression sequence [ 118 ]. On T2WI, they tend to show hyperintensity relative to the myocardium.…”

“…It is worth noting that because of the chemical shift effect, the black boundary sign in the cine sequence is helpful for the diagnosis of small lipomas. What is more, CMR can sensitively detect the myocardial infiltration of lipoma, which provides imaging evidence for treatment planning [ 118 ].…”

“…This can be obtained with a low-carbohydrate, high-fat diet and with the administration of 50 Units/Kg of body weight of unfractioned heparin before the injection of 18 FDG. 111,112 Other tumors, as neuroendocrine tumors and paragangliomas, require the use of different tracings as 68 Ga-DOTA-peptide or others. 113,114 For ventricular tumors, mostly if they are in close relationship with a coronary artery, a coronary angiography or a CT scan with cardiac synchronization and 3D reconstruction may be considered to better define both the size of the mass and its relationship with the neighboring structures; CT and MRI may also aid in the tissue characterization.…”

“…Rhabdomyomas, rhabdomyosarcomas, and leiomyosarcomas usually show a homogenous echogenicity; hyperechogenic and hyperlucent central areas may represent foci of necrosis or of calcification. Lipomas may show different aspects, from hypo‐ to hyperechoic, but are usually homogenous 67–69 . Fibromas show usually an increased echogenicity compared to the normal myocardium 70–72 .…”

“…On CT scans, lipomas are homogeneous fatty lesions with well-identified borders and attenuation values ranging between −30 and −100 Hounsfield units (HU), with no contrast-enhanced components. Compared with lipomas, liposarcomas are heterogeneous malignant fatty tumors, with a mean attenuation value greater than −30 HU, due to soft tissue and fibrous strands alternating with areas of fat [ 5 , 8 , 9 ]. On MRI, lipomas are homogenous lesions with signal intensity typical of fat tissue – hyperintense on both T1- and T2-weighted images, with the absence of internal septa or nodules and complete fat suppression on selective fat-suppressed images and showing little to no contrast enhancement.…”
