The value of ThinPrep and cytospin preparation in pleural effusion cytological diagnosis of mesothelioma and adenocarcinoma

Abstract: The diagnosis of malignant mesothelioma requires an integration of the clinical presentation, radiological studies, and immunohistochemical stain of histological sections. Cytological diagnosis on pleural effusions of mesothelioma and pulmonary adenocarcinoma is highly desirable but debatable. A spectrum of cytological features has been found to be associated more commonly with malignant mesothelioma (e.g., peripheral cytoplasmic skirt, bubbly cytoplasm, cyanophilic cytoplasm, and scalloped border of cell ball… Show more

“…However, the morphological analysis is not able to clarify all the doubts and answer about all the diagnostic queries. Allegedly, as highlighted by Ylagan et al, some morphological overlaps prevent from a conclusive diagnosis and they maximize the need for the selection of a panel of immunomarkers [17]. Several papers highlighted the improvements in the sensitivity and specificity of peritoneal washings from gynecological and gastro-intestinal cancers using a wide range of markers to define either a primary or a metastatic origin [2023].…”

“…In effusion specimens, the differentiation between mesothelioma and metastatic malignancies is oftentimes a challenge in the daily practice. One study compared the cytological features of confirmed malignant mesothelioma and pulmonary adenocarcinoma in pleural effusions with Diff-Quik-stained Cytospin and Papanicolaou-stained TP (table 1) [18]. This study suggested that the TP preparation of pleural effusions did not appear to provide additional diagnostic value compared with the Cytospin Diff-Quik preparation in distinguishing between mesothelioma and adenocarcinoma in pleural effusions.…”

“…Ylagan and Zhai [19] reviewed and compared 25 characteristic features in 15 mesotheliomas and 22 features in 13 ADC between Diff-Quik-stained CS and TP. For mesothelioma, bubbly and vacuolated cytoplasm, cell-in-cell arrangements and irregular nuclear contours were more frequently seen on CS, while enlarged nuclei were more frequently seen on TP (fig.…”
