The variation of serum cortisol during ovarian stimulation forin vitrofertilization

Abstract: Cortisol showed a progressive increase beginning from D1, especially after ovulation triggering, and returned to pre-treatment level next month. Cortisol variation was not related to the changes in the E(2) values. Cortisol values at both M-1 and D1 could be used as controls.

“…Nevertheless, FF cortisol levels were independent of peak serum E2 concentrations, consistent with the previous finding that circulating cortisol and E2 levels are unrelated to each other during ovarian stimulation for IVF (48). Yet, FF cortisol levels remained negatively correlated with cumulus cell lipid content, adjusting for patient age and numbers of oocytes retrieved, implying a role for cortisol in ovarian cellular lipid metabolism as shown in other lipid-containing cells (10, 11).…”

“…Some rational reasons for this can be mentioned here. First, previous studies have proven that the supraphysiologic levels of hormones such E 2 , P, and stress hormones (43,44) during the initial ovarian stimulation phase of IVF could lead to morphologic and biochemical modifications and consequently impaired endometrial receptivity with the use of fresh ET as well as affecting the early developing embryo (45), reducing the implantation rates and leading to increased risks of adverse perinatal outcomes (30,46). Next, there is still the increased risk for severe ovarian hyperstimulation syndrome (OHSS), which is a potentially life-threatening serious complication of ovarian stimulation in fresh cycles (47,48).…”

“…We have, also, took care of the possible adverse effect of anticonvulsant therapy -even before the 4 weeks prior the beginning of the study (34). A possible effect of the cortisol, due to high estrogen levels (35) was also avoided.…”
