The Versatile Facial Artery Perforator-Based Nasolabial Flap in Midface Reconstruction

Abstract: The nasolabial perforator flap has certain advantages such as the 1-stage application, repair using a similar tissue, a wider rotation arc around the pedicle compared to the other regional flaps, and the primary closure of the donor area. Based on these characteristics, it is an ideal alternative for the repairs of the defects located in the lower two thirds of the face or the intraoral region.

“…Also, the flap had limited mobility because of a short pedicle and a limited rotation angle. 9,10 Although the cheek was also a flap donor area, the available area of the buccal local flap was limited, and the scar left in the donor site was obvious. 11 Zhou reported his experience with orbicularis oculi myocutaneous flaps in the temporal area to repair eyelid defects caused by burns, cancer excision, or congenital deformity.…”

“…Durgeon et al advocated and popularized propeller type NLF for full thickness alar region defects. 6, 7 Transpositional NLF is mainly advocated in case of alar defects extending to rim and sidewalls. The technique to employ transpositional NLF is simple and quick as we have seen in this case report.…”

“…The facial artery perforator flap has shown promise for the reconstruction of cheek, paranasal, 19 periorbital, 20 and oral 20 defects. Centered on a thin perforator, the flap has a wide rotation arc and excellent mobility, permitting application in a wide variety of defects.…”

“…Modification of the flap to include the buccal mucosa, muscle, and skin can create a full-thickness flap for the repair of lip defects. 20 Like the cervical region, the submental region contains skin with similar color and texture to that of the cheek. A previous concern for the use of the submental region as a donor site is the proximity to the submental and submandibular nodes, notionally increasing the opportunity for reoccurrence of the tumor in the recipient site.…”
