The width of the common bile duct in childhood

Abstract: The maximum diameter of the common bile duct was measured on 85 normal intravenous cholangiograms carried out on children between the ages of 1--14 years. There was a close correlation between bile duct width and the patient's age, but a poor correlation with weight. The range of bile duct widths in each age group is tabulated as a guide to diagnosing biliary dilatation in childhood.

“…Furthermore, we previously reported cholangiocarcinoma in a child with choledochal cyst and AAPBD. 7 Based on these results, it was suggested that the extrahepatic bile duct should be excised in pediatric patients with the nondilated type of AAPBD as well as in those with the dilated type. 17 …”

“…7 In adult patients, however, it seems that there is a consensus definition of CBD diameter greater than 10 mm. 8 Miyano et al 9 defined a nondilated type CBD as less than 8 mm (patients aged 1 to 7 years), whereas Ando et al 10 defined a nondilated CBD as less than 6 mm (patients aged 1 to 6 years).…”

“…Adult studies have characterized the normal size of the EHD with several authors demonstrating a direct relation between patient age and the size of the EHD (1-3). By contrast, the available pediatric studies that have attempted to establish normal measurements of the EHD are limited in number, with the majority of these studies restricted by their small sample sizes, limited defined exclusion criteria, or inconsistencies in measurement protocol (4)(5)(6)(7)(8)(9).…”

“…In the adult population, the diameter of the normal EHD has been well-studied including studies correlating a gradual diameter increase for age in older healthy adults (1)(2)(3). In contrast, children have been studied to a much lesser extent, with prior studies being limited by small sample sizes and variable exclusion criteria (4)(5)(6)(7)(8)(9). Several studies have also limited the age assessment to young adolescents, whereas others have failed to include the youngest of age groups, specifically newborns and infants.…”

“…Therefore, to refer to a dilated duct, it is necessary to know the normal diameter of the common bile duct (choledochus). A study by Pina and average of 5-6 mm in diameter of the common bile duct in adult patients; however the diameter varies according to age and measurement method (Table 1) [1,2].…”
