Thoracic spine trauma: advanced imaging modality

Abstract: Technological advances in diagnostic imaging make the diagnosis of spinal trauma more accurate both in the acute and in the chronic settings. Improvements in medical care and in diagnostic imaging lead to an increasing prevalence of patients' surviving injuries. The management of these patients depends on early and appropriate radiological evaluation in the acute phase. The aim of this work was to review advanced imaging modalities employed to study thoracic spine and spinal cord in injured patients.

“…One of the most accredited theory, among others such as the inflammation and infection ones, is the biomechanical stress theory. 4. In our study, evaluating standard supine and weight-bearing MR scans of 38 patients with nonspecific low back pain and MRI evidence of Modic I vertebral changes, we found Modic I changes extent increase in the upright position in 26 patients (68.4%).…”

“…Degenerative disc disease of the spine causing low back pain is a leading cause of morbidity and chronic disability [1]. MRI is the preferred imaging technique to evaluate the degenerative changes of the spine [2,3], showing high sensitivity and specificity in the diagnosis of conditions like disc herniation, spinal stenosis, and spondylodiscitis [4][5][6][7]. In the last years, MRI signal changes in vertebral endplates, especially Modic changes (MC) type I, have been advocated to be a potential specific cause of low back pain [8,9].…”

“…16 Most spine fractures are associated with high-energy trauma or falls in the elderly. 14,17 At minimum, imaging should include AP and lateral radiographs of the entire spine, as well as an odontoid (or open mouth) view of the C-spine. 14,17 In the patient with acute trauma, it is acceptable to use CT imaging first, especially in unconscious or intermediate-to high-risk patients.…”

“…A limitation of CT scans is the beam-hardening artifact caused by the metallic prosthesis, which causes difficulty in the evaluation of the spinal canal; however, they are less common with titanium implants than with stainless steel because of the lower beam attenuation coefficient of titanium [30][31][32][33][34].…”
