Three-Dimensional Analysis of Retinal Microaneurysms With Adaptive Optics Optical Coherence Tomography

Abstract: Adaptive optics OCT imaging revealed that MAs located in the inner nuclear layer were connected to the intermediate and/or deep capillary plexus. Intraluminal hyperreflectivity seen on AO fundus camera images originated from a strong reflection from the vessel wall and only in a third of the cases from intraluminal clots. Currently, AOOCT is the most expedient in vivo imaging method to capture morphologic details of retinal microvasculature in 3D and in the context of the surrounding retinal anatomy.

“…Whereas the OCT signal highlighted several retinal layers, 2PM allowed us to retrieve the localization of CX 3 CR1 + cells, presumably tissue‐resident microglia based on their GFP fluorescence. These cells play a crucial role in the disease development in the chronically inflamed central nervous system , in general, and chronically inflamed retina , in particular, as shown by us and by others.…”

“…Whereas the axial resolution of OCT is typically higher than that of microscopy technologies including 2PM, its poor lateral resolution and poor contrast (due to the use of low NA lenses) are only able to determine information about organ structure at the tissue, and not at the single cell, level. In order to partially counteract this deficit, special experimental OCT setups have been developed and employed to retrieve skin architecture , the thickness of retinal layers , and to develop sensor‐free adaptive optics for in vivo imaging in the mouse retina . Still, OCT remains a technology characterized by poor lateral resolution.…”

“…12 With AO, visualization of changes in healthy and pathological aging has become possible in vivo on the subcellular level. However, up to now only a few patients have been imaged with AO-OCT [31][32][33] and no assessment of the detailed PR composition in patients with AMD is available. The following study presents insights into pathologically and healthy aged retinas as imaged with AO-OCT in 55 eyes of 32 participants.…”

“…13 However, we could recently demonstrate that AOOCT can reveal further details of MA morphology, which cannot be captured by these twodimensional AO techniques. 15 Here we conducted a systematic longitudinal imaging study with structured, regular AOOCT visits for a follow-up period of up to 21 months. Our results demonstrate that AOOCT imaging technology allows an accurate relocalization and follow-up of MAs to observe the highly dynamic and complex structural changes that occur in MA evolution with near-isotropic resolution in all three dimensions.…”

“…Though intraluminal clots were found in some MAs, most of the strong intraluminal reflections delineated with AO fundus camera imaging merely correlated to a thickening of the MA wall in the corresponding AOOCT volumes. 15 The purpose of the present study was to use AOOCT to prospectively observe retinal MAs in order to investigate changes of three-dimensional morphologic details in the natural history of their dynamic turnover.…”
