Three-dimensional Isotropic Functional Imaging of Cystic Fibrosis Using Oxygen-enhanced MRI: Comparison with Hyperpolarized 3He MRI

Abstract: To compare the performance of three-dimensional radial ultrashort echo time (UTE) oxygen-enhanced (OE) MRI with that of hyperpolarized helium 3 (3 He) MRI with respect to quantitative ventilation measurements in patients with cystic fibrosis (CF). Materials and Methods: In this prospective study conducted from June 2013 to May 2015, 25 participants with CF aged 10-55 years (14 male; age range, 13-55 years; 11 female; age range, 10-37 years) successfully underwent pulmonary function tests, hyperpolarized 3 He M… Show more

“…The whole lung VDP for 18 subjects (seven normal, five asthma, and six CF) was previously reported as part of an interim study to develop a postprocessing workflow for VDP quantification using OE MRI and compare VDP in diseased vs. normal groups . A comparison study on whole lung VDP measured from hyperpolarized 3 He vs. OE MRI for 24 subjects with CF was previously published …”

“…4 A comparison study on whole lung VDP measured from hyperpolarized 3 He vs. OE MRI for 24 subjects with CF was previously published. 28 3D Radial UTE OE MRI PULSE SEQUENCE. The 3D radial UTE sequence 29 was optimized to minimize TE and improve the signal-to-noise ratio (SNR) and image quality of lung imaging.…”

“…For example, the juxtaposition of patterns of bronchiectasis and air trapping visualized on UTE with semi-quantitative measures of regional ventilation defects from PSE maps depicts ventilation and perfusion defects local to areas of structural abnormality. This is seen in Figure 11, which exemplifies the ability to visualize the morphological and corresponding physiological abnormalities, such as ventilation defects comparable to those observed on hyperpolarized gas MRI (62), in a severe CF patient.…”

“…OE-MRI provides a potential alternative to hyperpolarized xenon MRI to assess regional obstruction in diseases such as CF (62). The complementarity of structure and function afforded by OE MRI with UTE can provide a framework for interpreting the functional severity of structural abnormalities in CF.…”
