Three-dimensional visualisation of coronary sinus ostium from the inside right atrium perspective

Abstract: Cardiac CT angiography is a useful method that permits the coronary sinus ostium and Thebesian valve to be visualised in vivo from the inside of the right atrium in a comparable manner.

“…24 To achieve the best outcome and to minimize the risk of complications, it is still recommended to try to visualize the coronary venous system prior to invasive interventions. 25 Coronary venous system anatomy is relevant not only during cardiac resynchronization therapy implantations but also during electrophysiology studies as well as ablation inside the venous system. Operators must always keep in mind that the oblique vein may be a site for inadvertent cannulation by guidewires, diagnostic electrophysiology catheters, and left ventricular pacing electrodes, posing a risk of coronary venous system perforation and pericardial effusion or tamponade.…”

“…In this study, we used transthoracic echocardiography to measure CS diameter at three sites. 3 Ominde et al measured CS diameter in cadavers and showed that the CS diameter decreased from the proximal to the distal part. 5 In our study, a decrease in diameter was also shown.…”

“…In adults, CS is a muscular tube, 2 to 3 cm in length and 1 cm in diameter, located about 1 cm above and parallel to the junction of the left atrium and ventricle. 1,3 Thebesian valve is a fold of endocardial tissue that forms the orifice of CS and is a remnant of the right embryonic sinoatrial junction valve. 4 Knowledge about the CS anatomy is essential due to its importance in different heart procedures such as percutaneous mitral annuloplasty, retrograde cardioplegia, and biventricular pacing.…”

“…Despite its great clinical significance, the presence of the VV cannot usually be assessed preoperatively, because standard clinical resources seem to not allow the visualization of intravenous heart valves . In our unique study, we aimed to determine if the MSCT could fill this void.…”
