The usage of laser in medicine is not recent, and its history in urology goes back to 40 years ago. For the last 2 decades, common uses of laser have been treatments of subjects with urolithiasis, bladder tumors, benign prostatic enlargement, le sions of the genitalia and urinary tract strictures. To evaluate laser application in urology in Iran, we reviewed all of the Iranian literature on the topic. This study was designed to retrieve all studies on laser application in urology in Iran, regardless of publication status or language, covering years 1990-2017. Twentysix articles were identified: 12 about urolithiasis, 8 about benign prostatic hyperplasia (BPH), 2 case reports, 1 paper about prostate cancer, 1 on female urethral stricture, 1 review and 1 basic sciences study. We conclude that the use of this technology has not yet found its position in Iran, especially in the field of urology. The main causes for it are the difficult accessibility and disturb of laser devices and its accessories, as well as the lack of adequate knowledge of the medical community about this modality.

StreszczenieWprowadzenie i cel pracy. Lasery pełnią ważną rolę w diagnostyce i terapii urologicznej. Ich znaczenie i możliwość zastosowania w leczeniu schorzeń układu moczowego rośnie wraz z rozwojem technik endoskopowych. Celem pracy jest przedstawienie aktualnych możliwości terapeutycznego zastosowania laserów w urologii. Skrócony opis stanu wiedzy. Obecnie w urologii najszersze praktyczne zastosowanie znajdują cztery rodzaje laserów: laser holmowy (Ho:YAG), laser tulowy (Tm:YAG), laser potasowo--tytanowo-fosforanowy (KTP) oraz laser diodowy. Różnią się one przede wszystkim ośrodkiem czynnym, który determinuje długość fali, a także trybem pracy (ciągły lub pulsacyjny) i mocą. Przekłada się to na istotne różnice w oddziaływaniu na tkanki, głębokości penetracji wiązki lasera oraz możliwości zastosowania poszczególnych laserów w wybranych zabiegach. W leczeniu łagodnego rozrostu gruczołu krokowego zastosowanie znajdują wszystkie wymienione lasery. W zależności od stosowanego lasera różna jest technika i zakres zabiegu -możliwa jest enukleacja, waporyzacja lub resekcja prostaty. Ponadto laser holmowy jest wykorzystywany w leczeniu kamicy moczowej i jest bardzo skuteczną alternatywą dla litotrypsji falą generowaną pozaustrojowo (ESWL). Zastosowanie laseroterapii w onkologii urologicznej jest obecnie przedmiotem licznych badań. Podsumowanie. Liczne zalety laserów w terapii schorzeń urologicznych są szczególnie widoczne w zabiegach endourologicznych. Często cechują je większe bezpieczeństwo i mniejsza liczba powikłań w porównaniu do klasycznych metod leczenia przy zachowaniu podobnej lub nawet wyższej skuteczności. Lasery w urologii stopniowo zastępują dotychczas stosowane techniki lub stanowią dla nich dobrą alternatywę.
