Thyroid Lobectomy for Papillary Thyroid Cancer: Long‐term Follow‐up Study of 1,088 Cases

Abstract: The long-term clinical outcome of the PTC patients who had been treated by lobectomy without RAI ablation was excellent. Based on the above results, we concluded that lobectomy is a valid alternative to total thyroidectomy for the treatment of PTC patients who are younger than aged 45 years, whose tumor diameter is 40 mm or less, and who do not have clinical lymph node metastasis or extrathyroidal invasion.

“…Similar findings were reported by Memorial Sloan Kettering in a matched pair analysis [20]. Single institutions would provide increasingly compelling evidence that more aggressive therapy was not associated with improved outcome [21][22][23][24]. These findings were supported by updated national database studies, which also concluded that a more aggressive approach could not be supported in low-risk groups [25].…”

“…How to include age as a prognostic factor in PTC is still a matter of debate, and previous studies have adopted various cutoffs as shown in Table 1 [3][4][5][6][7][8][9]. These studies' findings indicate that elderly patients are more likely to show recurrence and die of carcinoma compared to young patients.…”

“…Matsuzu et al also demonstrated that TS > 4 cm is one of the risk factors of carcinoma death [4]. Most studies set the cutoff at 3-4 cm according to the prognosis of patients (Table 2) [1,3,4,5,7,8,20].…”

“…AMES has marked tumor size more than 5 cm as high risk and UICC set two cut of at 2 and 4 cm [25,27]. Most of the recent studies have set the cut of at 3-4 cm according to prognosis of PTC patients [25,[27][28][29][30]35]. …”
