Thyroid nodules with indeterminate cytology: prospective comparison between 18F-FDG-PET/CT, multiparametric neck ultrasonography, 99mTc-MIBI scintigraphy and histology

Abstract: Purpose: To evaluate the role of 18 F-flurodeoxiglucose positron emission tomography/computed tomography ( 18 F-FDG-PET/CT) in predicting malignancy of thyroid nodules with indeterminate cytology. Patients and methods:We analysed 87 patients who have been scheduled to undergo surgery for thyroid nodule with indeterminate cytology. All patients underwent 18 F-FDG-PET/CT, multiparametric neck ultrasonography (MPUS), and 99m Tc-methoxyisobutylisonitrile scintigraphy ( 99m Tc-MIBI-scan). Histopathology was the sta… Show more

“…The lack of reliable data in this clinical setting motivated us to evaluate 18 F‐FDG PET/CT in a well‐selected and larger population of patients. The present data, properly confirmed with histology, were in line with those reported for nodules with indeterminate cytology . Particularly, when we evaluated the performance of 18 F‐FDG PET/CT in the subgroup of patients with indeterminate FNAC we found results quite similar to that previously published with high NPV .…”

Particularly, when we evaluated the performance of 18 F‐FDG PET/CT in the subgroup of patients with indeterminate FNAC we found results quite similar to that previously published with high NPV . In other words, the risk of malignancy in FDG positive nodules remains fairly low but is clearly reduced in FDG negative lesions.…”

“…Indeed, 14 of the 75 18 F‐FDG avid thyroid nodules were suspected at US (EU‐TIRADS 5) and 11/14 were histologically proven cancers. These results corroborate those presented in another diagnostic context such as thyroid nodules with indeterminate FNAC report; in that study, it was proved that the diagnostic pattern of a positive 18 F‐FDG thyroid nodule with suspected US characteristics was significantly more specific for malignancy than that of a 18 F‐FDG alone …”
