Thyroid Tuberculosis Mimicking Carcinoma: Report of Two Cases

Abstract: Among 527 patients with thyroid disease who underwent surgery at our hospital during a 20-year period, 2 (0.4%) had tuberculous thyroiditis mimicking carcinoma. The first patient was a 44-year-old man with a solitary thyroid nodule and the second was a 24-year old man with a thyroid abscess. The unexpected diagnosis was made postoperatively and was based on histological findings in both patients. No primary focus was found elsewhere in either patient, and both responded to antituberculous chemotherapy. Althoug… Show more

“…There is also a rare isolated variety of thyroid tuberculosis that may give rise to a solitary nodule mimicking carcinoma, cold abscess with discharging sinus, diffuse goiter, or, very uncommonly, an acute abscess. 1 The clinical presentation of thyroid tuberculosis is nonspecific and the diagnosis is usually a surprise at pathological examination. 2,3 An accurate diagnosis of thyroid tuberculosis can avoid unnecessary thyroidectomies.…”

“…20 In the event that pain is absent, tuberculous thyroiditis can often be mistaken for carcinoma. [21][22][23] It is important to note that tuberculous thyroiditis may coexist with thyroid carcinoma in the same patient. [24][25][26] The diagnosis is mainly made by ultrasound guided fine-needle aspiration cytology, a diagnostic step that helps to avoid unnecessary surgery.…”

“…low density centre and thin margin of high density evoked pyogenic membrane) resembled a retropharyngeal abscess. Several cases of tuberculous granuloma in thyroid gland imitating cancer have been published (1,2,3). Cases of anaplastic thyroid cancer, or more precisely lymph node metastasis of thyroid cancer, resembling a retropharyngeal abscess, are rare.…”
