Time‐dependent benefits of pre‐treatment with new oral P2Y12‐inhibitors in patients addressed to primary PCI for acute ST‐elevation myocardial infarction

Abstract: Objectives The aim of this observational study was to determine the benefits of the novel, orally delivered P2Y12‐inhibitors (Is) in terms of angiographic endpoints and in relation to the time of the loading dose (LD) administration. Background The goal of ST‐elevation myocardial infarction (STEMI) treatment is timely reperfusion. The P2Y12‐Is prasugrel and ticagrelor have improved the angiographic outcome of primary percutaneous coronary intervention (pPCI) and patients' prognosis. However, their onset of act… Show more

“…In this issue of Catheterization and Cardiovascular Interventions , Pepe et al add to the limited evidence base with a unique analysis of 328 eligible consecutive STEMI patients referred to their center for primary PCI. Unlike other studies where the delays to PCI are minimal, this study included patients that were transferred from other facilities, allowing for analysis of the time dependence of P2Y12 inhibition.…”

“…In fact, limitations of Clopidogrel, Prasugrel, and Ticagrelor (all oral P2Y 12 -I) are the slow onset and offset of action and the impossibility to be administrated or to be fully effective in patients with orotracheal intubation, vomit, and impaired intestinal absorption 5 . In detail, the extremely short time gap between first medical contact (FMC) and primary PCI jeopardizes the effectiveness of the administration of oral P2Y 12 -I in ST elevation myocardial infarction (STEMI) patients, while in the setting of non-ST elevation (NSTE) ACS the administration of an oral P2Y 12 -I prior to coronary angiography (pretreatment strategy) is discouraged by the current ESC guidelines 1 , 6 .…”

“…Effectiveness of reperfusion strategies as well as of supportive therapies, such as P2Y12 inhibitors, has been proved, over the years, to be strictly time-dependent. 8 As a consequence, the ideal timing of SP16 must be a further crucial object of upcoming investigations.…”
