Tips and Tricks for Better Endoscopic Treatment of Colorectal Tumors: Usefulness of Cap and Band in Colorectal Endoscopic Mucosal Resection

Abstract: Endoscopic mucosal resection (EMR) is an endoscopic alternative to surgical resection of mucosal and submucosal neoplastic lesions. Prior to the development of knives, EMR could be performed with accessories to elevate the lesion. After the development of various knives, en bloc resection was possible without other accessories. So, recently, simple snaring without suction or endoscopic submucosal dissection using knife in the epithelial lesions such as adenoma or early mucosal cancer has been performed. Howeve… Show more

“…R-NETs commonly appear as submucosal lesions with yellowish mucosa [ 14 , 15 ]. There is still controversy on the treatment of R-NETs, and numerous treatment strategies have been reported [ 7 , 16 ], including EMR, EMR-c, EMR-l, ESD, etc. In this study, we retrospectively analyzed the safety and effectiveness of EMR-c and ESD in R-NETs ≤ 15 mm, and predicted the risk factors of incomplete resection.…”

“…The 'Cap and band' method was reported to be used in colon mucosal lesion resection [9] . With sufficient aspiration and band ligation performed using a cap-mounted endoscope, completely ligated, all-layers circular-resected specimens can be obtained without residue tumor.…”

“…The cap-assisted EMR (C-EMR) and EMR with a ligation device (EMR-L) in the colon have limited indications, especially for R0 resection of small rectal neuroendocrine tumors (NETs) because their radical removal can be difficult to achieve with standard endoscopic resection techniques due to the frequent involvement of the submucosal layer[ 76 , 77 ].…”
