Este documento analiza el efecto de la regulación en los procesos de innovación de la industria farmacéutica. El objetivo es analizar el efecto que provoca la regulación en el desarrollo de un medicamento innovador, llevado a cabo por una empresa farmacéutica de capital mexicano. La investigación es de tipo exploratorio y el diseño utilizado es un estudio de caso. El caso es el desarrollo de un nuevo medicamento para tratar la diabetes. La información se recabó principalmente mediante entrevistas semiestructuradas a personal de la empresa, que participó en el proyecto de innovación. Para entender el efecto de la regulación se partió del análisis de los requisitos regulatorios que tuvo que satisfacer la empresa. Se analizaron las diferentes etapas del proceso de innovación, y se encontró que la mayoría de los efectos se concentran en tres etapas: (i) la etapa donde se involucran seres humanos para el desarrollo de pruebas (etapa de ensayos clínicos), (ii) la etapa de registro sanitario, donde se verifica que el medicamento a comercializar cumple con las condiciones de seguridad, eficiencia y calidad, y (iii) la etapa de manufactura, para garantizar la idónea fabricación del medicamento según las buenas prácticas. La normatividad generó tanto efectos positivos como negativos. Entre los efectos positivos, destacan la regulación sobre propiedad intelectual y sobre la calidad del medicamento. Los principales efectos negativos se relacionan con la implementación de la regulación, más que con las exigencias que esta impone. A lo largo del proceso de innovación la regulación generó retrasos en las actividades planeadas, lo que conllevo a sobrecostos, pérdidas de competitividad e incertidumbre sobre el retorno de la inversión.

Background Excessive Gestational Weight Gain (EGWG) and anxiety are co-morbid conditions that increase the risk of adverse maternal and neonatal outcomes. This study was conducted to investigate the effect of yoga on the anxiety of women with excessive pregnancy weight gain. Material and methods This randomized controlled trial was performed on EGWG pregnant women referring to comprehensive health centers in Qom city, Iran, between October 2021 and August 2022. Eighty-eight participants were assigned to the intervention (N= 44) and control group (N=44). The experimental group participated in six sessions of ninety-minute yoga classes, and the control group only received routine care. Two questionnaires including a demographic information questionnaire, and the State-Trait Anxiety Inventory (STAI) questionnaire were used for data collection. Data were analyzed using SPSS software version 22. Results The results of this study showed a statistically significant difference between the two groups in terms of trait anxiety (25.84±3.45 vs. 57.38±8.07; p<0.05) and state anxiety (27.93±3.72 vs. 60.13±8.13; p<0.05) after the intervention. On the other hand, the trait and state anxiety rate were stable in the experimental group before and after intervention, while they increased to the severe form of anxiety in the control group (effect size = -21.84±10.66 vs. - 19.43± 8.44). Conclusion The result of this study showed that yoga has a positive effect on the anxiety of pregnant women with EGWG, and can be used as a preventive or complementary solution to control the anxiety of these mothers.

Trans individuals experience an incongruence between their sex characteristics and their gender which might cause significant distress called gender dysphoria. Gender dysphoria is commonly treated using transition‐related healthcare services, like sex hormones and surgeries, ideally in interdisciplinary settings. The stakeholder environment of an interdisciplinary transgender healthcare centre (ITHC) is considered key in delivering high‐quality healthcare. Therefore, a stakeholder analysis was performed seeking to improve stakeholder relationships and service quality through an evidence‐based approach. Quantitative data were collected using a questionnaire administered cross‐sectionally, and covering attitude towards the ITHC, influence of and influence on stakeholders, level of knowledge about the ITHC, importance of the ITHC for stakeholders and types of interests. The analysis used primary data, collected July to August 2015. N = 79 key stakeholders were identified and n = 42 (53.2%) participated in the survey. Participants were categorised analytically into four stakeholder groups: trans groups, trans health experts, healthcare system and admin departments of the ITHC. Although participants reported low values (e.g. average values on a 5‐point Likert scale; importance: 2.54; influence on the ITHC: 2.43), the attitude of the four stakeholder groups towards the ITHC were positive overall. The attitudes varied, however, depending on the group, for example trans health experts and trans groups reported the highest values for most items. The results demonstrate the importance of systematically analyzing stakeholder positions in order to make appropriate policy decisions, improving stakeholder relationships and assuring long‐term high‐quality healthcare provision. Subsequently, an action plan was developed, focusing on the two groups with the highest values (trans health experts, trans groups). Selected measures are discussed. If stakeholders are to play their part in providing high‐quality, interdisciplinary trans healthcare, they need regular information on the latest developments and recurring feedback of their interests and requirements for the ITHC.
