Aim Heart failure (HF) and chronic obstructive pulmonary disease (COPD) are a common comorbidity. Professional chronic obstructive pulmonary disease (PCOPD) is a specific phenotype, which suggests peculiarities in the development of HF. Difficulties of HF diagnosis in such patients determine the relevance of searching for additional markers. The aim of the study was identifying HF markers in patients with PCOPD.Material and methods This single-site, cohort, prospective, observational study included 345 patients. The main group consisted of PCOPD patients; the comparison group consisted of patients with COPD induced by tobacco smoking; and the control group included conventionally healthy individuals. The groups were matched by the index of coincidence; pairs were matched at 1:1 by the “nearest neighbor index”; covariates for matching included COPD duration, sex, and age. Each group included 115 patients. The major professional adverse factors were silica-containing dust and organic solvents. COPD was diagnosed according to GOLD criteria; HF was diagnosed in accordance with Russian clinical guidelines. The markers were determined by multifactorial logistic regression. Likelihood of events with allowance for the time to the event was analyzed by the Kaplan-Meier method.Results HF in PCOPD patients was characterized by biventricular damage, preserved left ventricular ejection fraction, and frequent hospitalizations for decompensation (17.5 % vs. 9.5 % for COPD in smokers). HF markers in patients with PCOPD included the length of work of more than 20 years, pulmonary artery systolic pressure (PASP) higher than 35 mm Hg according to data of Doppler echocardiography, diffusing capacity of lungs for carbon monoxide (DLCO) less than 50 %, increased serum concentrations of CC-chemokine ligand 18 (CCL18), S-100‑beta protein, and N-terminal pro-brain natriuretic peptide (NT-pro-BNP). Diagnostic sensitivity of the multifactorial model was 84 % and specificity was 81 %. Two models were proposed for purposes of screening, which included the following parameters: length of work, exposure to aromatic hydrocarbons, decreased distance in 6-min walk test by more than 60 m per year and length of work, exposure to inorganic dust, and decreased forced expiratory volume during the first second by more than 55 ml per year.Conclusion The markers for development of HF in PCOPD patients are length of work >20 years, PASP >35 mm Hg, DLCO <50 %, and increased serum concentrations of CCL18, S-100‑beta protein, and NT-pro-BNP.

A great share of workers employed at polymetallic ores mining have to face harmful working conditions at their workplaces. To provide safe working conditions for them and to preserve their health is a vital task occupational medicine has to tackle. Polymetallic ore mining enterprises employ certain common technological processes; nevertheless, there are specific features depending on ore mining techniques and the mineralogical composition of different ores. These features determine differences both in working conditions and in occupational risks of developing occupational morbidity (OM) and work-related morbidity (WRM). By now, there have been enough studies on peculiarities of occupational health risks for workers employed at sulfide ore mines, copper-nickel ore mines and ferruginous quartzite mines. Considerably less attention has been given to assessing occupational risks for workers dealing with mining and processing copper-zinc ores. We performed complex clinical and hygienic examinations at a major copper-zinc ore mining enterprise located in the Southern Urals. The research results gave grounds for determining a category of working conditions, establishing formation peculiarities and the structure of occupational and work-related diseases among workers from various occupational groups. Occupational risks were assessed considering hygienic and medical-biological indicators. We established the highest occupational health risks for shaft sinkers, followed by drilling unit operators, timberers, excavator drivers, load haul dumper (LHD) operators and underground self-propelled machine (USPM) operators. The research results gave grounds for developing a conceptual model of assessing and managing occupational risks in the branch. The urgency of developing and implementing activities aimed at risk mitigation should be determined depending on how validated a risk is and on its rates established for specific occupational groups.

For many years, morbidity with work-related diseases has been higher in Chukotka Autonomous Area (ChAA) than on average in Russia. Our research objects were working conditions and morbidity with work-related diseases. Our research goal was to examine reasons for work-related pathology occurrence, its structure, and prevalence in ChAA in 2008–2018. We examined data obtained via social-hygienic monitoring in 2008–2018 in Chukotka, paying close attention to a section entitled «Working conditions and work-related pathology». We established that noise (17.4%) and cooling microclimate (11.8%) were the most widely spread hazardous occupational factors in ChAA. 20.1% workers were exposed to a combination of hazardous factors. 13.5% workers were employed at industrial objects that belonged to the 1st surveillance group (the highest risks); 31.9% worked at economic entities from the 3rd surveillance group (average risks). In 2008–2018 216 work-related diseases were first diagnosed in Chukotka, mostly among workers employed at mining enterprises (81.5%). Sensorineural hearing loss / noise effects in the internal ear (35.2%) and respiratory diseases (31.9%) prevailed in their structure. 73.6% diseases were detected due to patients applying for medical aid themselves. In 2008–2015 there was a steady growth in work-related morbidity (from 1.94 to 13.5 per 10,000 workers), but there was a decrease in it in 2016–2018 (down to 5.11 per 10,000 workers) with considerable fluctuations in numbers of first diagnosed diseases. Risks of work-related pathology occurrence were higher in Chukotka in 2018 than in 2008: OR=2.37; CI 1.82–3.09; χ2=43.8; р<0.001. To prevent work-related pathology in Chukotka, it is necessary to continue activities aimed at working conditions improvement, in particular, reducing exposure to noise and aerosols with predominantly fibrogenic effects in mining industry in the region.
