Topographical Electrogastrograms After Radical Esophagectomy with Colonic Replacement

Abstract: The colonic and gastric EGG activities had 3-cpm, and probably 6-cpm in common. However, the colonic EGG activities were significantly different from the gastric EGG activities in frequency in the 3-cpm group, and in amplitude in both the 3- and 6-cpm groups. Thus, the replaced colon seems to preserve the original colonic EGG activity.

“…The origin of the EGG activity below 2.4 cpm seems still not clear, however, the bases of the colonic origin are as follows. The spectral frequencies and instability factors of the 1 cpm group of the colon replacement were higher than the preoperative controls (Homma et al, 2003) as shown in this study. The 24-hour ambulatory pressure waves in the replaced colon were found to be largely in the 0-2.4 cpm component (Watanabe et al, 2004).…”

“…In other words, the spectral frequency of the gastric myoelectric activity of EGG, overlaps the colonic frequency (Amaris et al, 2003). The isopower mapping or topographical EGGs based on power amplitudes of five spectral groups helped to visualize the above concept and the position of the stomach, the small intestine and the colon in a dorso-ventral plane (Homma, 1997(Homma, , 2005Homma et al, 1999;2003). “…There was a wide range of both ages and the time since surgery in the patients, depending on the type of surgery; TG (67.6 ± 11.8 years old, 63.0 ± 67 postoperative months, n=9) due to gastric cancer, TC (39 ± 13 years old, 43.3 ± 29.3 postoperative months, n=10) due to ulcerative colitis , DG (Birlloth I, 72 ± 3.3 years old, 115 ± 31.7 postoperative months, n=12) due to gastric cancer, and CR (without residual stomach: 66.2 ± 4.3 years old, 64.0 ± 33.7 postoperative months, n=5) due to esophageal and gastric cancer. Previous data from the 27 locations were added to the analysis of the superimposed topographic EGG maps for clearer demonstration of the control and the surgery-associated differences, including a TG (n=7), and TC (n=7) (Homma et al, 1999) and DG (n=12, after Homma et al, 2008 and n=10, unpublished data) and CR (n=6) (Homma et al, 2003). Therefore, the total case number was 16 for TG, 17 for TC, 22 for DG and 11 for CR (Fig.…”

“…Local differences are observed in the topograhic electrogastrographic (EGG) maps recorded at 27 different thoraco-abdominal locations in normal control subjects as well as in subjects who have undergone total gastrectomy (TG), total colectomy (TC), distal gastrectomy (DG) and colonic replacement (CR) surgery (Homma et al, 1999;Homma et al, 2003;Homma, 2005;2009).
