Total atrial conduction time to predict occult atrial fibrillation after cryptogenic stroke

Abstract: Our data suggest that measurement of TACT could help to predict future AF detection in patients with cryptogenic stroke. The clinical importance of prolonged rhythm monitoring or indication of direct anticoagulation therapy after cryptogenic stroke based on TACT should be further investigated.

“…Longitudinal strain-and strain rate analyses of the LA were performed offline (QLab 7.0, Philips Medical Systems, USA) using zoom mode images of the LA in four and two chamber views. The interval between the onset of P-wave in lead II of the ECG on echocardiographic images and the peak A`-wave of the lateral mitral valve (MV) annulus in tissue Doppler imaging was defined as PA-TDI lateral (lPA-TDI) [18,19]. Furthermore, we measured this parameter at the septal MV annulus and defined the parameter as PA-TDI septal (sPA-TDI).…”

“…LA maximal volume (HR 1.07), total LA ejection fraction (HR 0.96), and PA-TDI duration (HR 1.05) were independent predictors of new-onset AF in patients after acute myocardial infarction [66]. Moreover, PA-TDI > 145 ms (93.8% sensitivity and 90.5% specificity) was shown to predict AF in patients with cryptogenic stroke [67]. Also, intra (left) atrial mechanical delay obtained with simultaneous recording of Doppler waveforms at two different sites (tissue Doppler echocardiography) may predict AF recurrence [68].…”

“…Another investigation found that the total atrial conduction time measured by Doppler echocardiography using the PA‐TDI interval predicts hidden AF in patients with CS with a sensitivity of 93.8% and specificity of 90.5%. After a multivariate analysis, the last parameter was an independent predictor of hidden AF …”

“…After a multivariate analysis, the last parameter was an independent predictor of hidden AF. 14 There are other proposed schemes for prediction of paroxysmal AF following an ischemic stroke. One of the best one studied is the iPAB score, which includes a personal history of arrhythmia or antiarrhythmic drug use, atrial dilatation, and elevation of brain natriuretic peptide.…”
