Transanal polypectomy using single incision laparoscopic instruments

Abstract: Transanal excision of rectal polyps with laparoscopic instrumentation and a single incision laparoscopic port is a novel technique that uses technology originally developed for abdominal procedures from the natural orifice of the rectum. Transanal endoscopic microsurgery (TEM) is a well established surgical approach for certain benign or early malignant lesions of the rectum, under specific indications. Our technique is a hybrid technique of transanal surgery, a reasonable method for polyp resection without th… Show more

“…Some have also raised concern about post-TAMIS faecal incontinence with use of the TAMIS 40 mm rigid proctoscope [ 16 ]. Many of these concerns were overcome by using the SILS port (Covidien, Mansfield, MA, USA) to perform TAMIS [ 17 , 18 ]. However, this is a limitation in a resource poor environment such as ours.…”

“…Lesions most successfully treated by TAMIS include all lesions (<3 cm) that are typically limited to <30% of the bowel wall circumference. While 3 cm is the recommended size criteria for lesions amenable for TAMIS removal, a small case series demonstrating the success of surgical resection >3 cm has been recorded [ 5 , 6 , 7 , 8 , 9 , 10 ]. TEM and TAMIS involve local excision of the tumor with a full-thickness excision containing the mucosa, submucosa, and muscularis propria.…”

“…Transanal excision of rectal adenomas via a laparoscopic single access port device was described for the first time in 2010 by Khoo et al andDemirbas et al (accepted 2010, published 2012) [13,14]. Several case reports [15,16] and case series followed, investigating different laparoscopic single port devices [10,12,[17][18][19].…”
