Transcatheter Arterial Embolization (TAE) in the Management of Bleeding in the COVID-19 Patient

Roberto Minici,
Federico Fontana,
Massimo Venturini
et al.

Abstract: Background and Objectives: Increasing attention is being paid to the coagulation disorders associated with SARS-CoV-2 infection. Bleeding accounts for 3–6% of COVID-19 patient deaths, and is often a forgotten part of the disease. The bleeding risk is enhanced by several factors, including spontaneous heparin-induced thrombocytopenia, thrombocytopenia, the hyperfibrinolytic state, the consumption of coagulation factors, and thromboprophylaxis with anticoagulants. This study aims to assess the efficacy and safet… Show more

“…Postoperative hemostasis maintenance can be indirectly assessed by bleeding-related VASCs, which can serve as surrogate markers [ 18 ]. In our investigation, bleeding-related VASCs were observed in seven (5.8%) cases and VASCs in 10 (8.3%) cases, in keeping with the current literature [ 56 , 57 ]. Therefore, manual compression closure of the femoral arterial access site aided by the Axiostat ® hemostatic dressing proved to be safe in patients undergoing endovascular treatment.…”

“…For these reasons, the ideal treatment for carotid blowout syndrome remains controversial and dependent on operator preferences [ 51 ]. Finally, in our investigation, the safety results, encompassing VASCs, demonstrate an alignment with previous studies within the realm of endovascular interventions and TAEs [ 67 , 68 , 69 , 70 , 71 , 72 , 73 , 74 , 75 , 76 , 77 , 78 , 79 ].…”

“…In contrast, other reports have shown similar rates of TAE success and rebleeding between patients with coagulopathy and those without [ 19 ]. Additionally, the coronavirus disease 2019 (COVID-19) pandemic has resulted in increased reports on effective conservative management of spontaneous major bleeds in patients with coagulopathy [ 45 , 46 , 85 ]. Therefore, data on TAE in patients with coagulopathy are controversial, and the management of major acute bleeding in patients with coagulopathy is still debated.…”
