Transgastric endoscopic gallbladder polypectomy and cholecystolithiasis: A case report

Abstract: As the most common digestive-system disease, cholelithiasis and gallbladder polyps have a high incidence. The most common treatment is laparoscopic cholecystectomy, but there are numerous drawbacks, including stump syndrome. In the present study, a novel treatment, namely transgastric endoscopic gallbladder polypectomy and cholecystolithiasis, was applied. To the best of our knowledge, the present study is the first to report on this application, which can potentially avoid open surgery and associated scars an… Show more

“…In 2015, Liu completed TR-GPP and gallstones extraction [ 35 ], which pioneered endoscopic gallbladder-preserving polypectomy procedures. Subsequently, TG-GPP also emerged [ 36 , 37 ]. In a study that included 22 patients [ 37 ], 12 of whom underwent TG-GPP for polypoid lesions, four patients suffered localized peritonitis (4/22, 18.2%) after the procedure, which subsequently recovered after conservative medical treatment.…”

“…Trans-gastric access: In 2020, Li and Han and in 2022, Zhang et al reported the transgastric NOTES procedure. [ 25 , 26 ] The main steps for transgastric NOTES cholecystolithotomy include stomach cleansing with sterile saline solution, insertion of a disinfected gastroscope with a transparent cap attached on the tip of the endoscope and a longitudinal incision on the greater curvature with Hook/IT knives, and endoscope entry into the peritoneal cavity. The rest of the steps were similar to that of transrectal NOTES.…”

“…The incidence rate of cholelithiasis has increased from 10% to 20% in recent years [1]. Transgastric approach of gallbladder preservation surgery refers to With the help of transparent cap, a full-thickness endoscopic incision was made in the posterior antrum near the lesser curvature of the stomach and the anterior wall of the lower part of the gastric body with hook knife and its knife [2]. After full-thickness resection, the greater omentum in the abdominal cavity was separated layer by layer, and the passage between stomach and abdominal cavity was opened, Then the gallbladder was found in the abdominal cavity, and a small incision was made at the junction of gallbladder with hook knife [2].…”

With the same tool, the window was enlarged to allow the endoscope to enter the gallbladder, and the gallbladder was pulled by the clip with inner ring, so that the endoscope could enter the gallbladder easily.…”
