Transient Elastography (FibroScan) Is Not Correlated With Liver Fibrosis but With Cholestasis in Patients With Long‐Term Home Parenteral Nutrition

Abstract: In patients with long-term HPN, TE failed to assess the degree of hepatic fibrosis. This could be due to the heterogeneity of liver histologic features observed in these patients and the presence of chronic cholestasis.

“…The presence of IFALD varied in the included studies, partly because different definitions of IFALD were used, with the different index tests focusing on different aspects of lFALD (Tables and 2). Steatosis was present in 35%–53% of the patients, cholestasis in 7%–37%, and fibrosis in 70%–93% (Figure ). The percentage of patients who had hepatic inflammation could only be extracted from 1 study (13%) …”

“…Of the other 15 studies that were potentially eligible, 2 performed no diagnostic assessment, 4 performed no other test than conventional liver biochemistry tests, and 5 did not include a reference test. In the end, we identified 4 studies fulfilling the inclusion criteria for our review (Figure ) …”

“…The principle confounders of VCTE are inflammation, cholestasis and BMI. Inflammation and cholestasis, reflected respectively by the alanine aminotransferase (ALT) and alkaline phosphatase level, are important, independent determinants of liver stiffness [18][19][20]. There are no explicit BMI limits for VCTE, though LSM success declines, especially above 35 kg/m 2 .…”
