Transient leukopenia, thrombocytopenia, and severe neutropenia associated with acute SARS‐CoV‐2 infection

Abstract: LETTER TO THE EDITOR further evaluation with a bone marrow biopsy or requiring therapeutic intervention. Bone marrow biopsy may not be indicated in patients with SARS-CoV-2 infection and hematological abnormalities given the transient nature of blood cell lines even in those with trisomy 21. ACKNOWLEDGMENTSThe authors would like to acknowledge the multidisciplinary team at Lehigh Valley Reilly Children's Hospital. “…According to European Society of Clinical Microbiology and Infectious Diseases (ESCMID) guidelines, it is recommended to use tocilizumab in hospitalized patients with severe COVID-19 24 . A study conducted in cancer patients indicated that higher COVID-19 severity was associated with abnormal CRP levels 25 . Another randomized clinical trial on tocilizumab reported that patients with CRP levels above 150 mg/L benefited from tocilizumab treatment 26 .…”

“…SARS-CoV-2 could directly affect hematopoietic cells and bone marrow stromal cells inducing thrombocytopenia. 75 Bacterial sepsis in hospitalized patients may hamper the expression of plasminogen activator inhibitor-1 (PAI-1), therefore, fibrinolysis is interrupted, and more vigorously the number of PLTs will drop in Covid-19 patients. Persistent hypertension, low oxygen levels, and diffused alveolar damage alters the integrity of pulmonary capillary beds and suppresses PLT release.…”

“…Destruction of the bone marrow, commonly seen in hematological diseases, might lead to leukopenia as the production of leukocytes is located in the bone marrow. Further reasons for leukopenia include vitamin deficiency (especially vitamin B 12 ), folic acid and copper deficiency, toxic and radiation damage, as well as autoimmune diseases and infectious diseases (such as HIV and SARS-CoV-2) [ 10 , 11 , 12 ]. Several drugs are known to be associated with leukopenia by inducing apoptosis, inhibiting leukocyte development, or being toxic for myeloid precursor cells [ 13 ].…”
