Transurethral seminal vesiculoscopy for recurrent hemospermia: experience from 419 cases

Abstract: We summarized our experience in transurethral seminal vesiculoscopy (TSV) for recurrent hemospermia by introducing surgical techniques, intraoperative findings, and treatment outcomes. TSV was performed in 419 patients with an initial diagnosis of persistent hemospermia at Shanghai Changhai Hospital (Shanghai, China) from May 2007 to November 2015. TSV was successfully performed in 381 cases (90.9%). Hemospermia was alleviated or disappeared in 324 (85.0%) patients by 3 months after surgery. Common intraoperat… Show more

“…PI-RADS could diminish variation in the interpretation and reporting of prostate imaging, especially among readers with varied experience levels [69]. Multiple studies has also proved mpMRI based on PI-RADS was useful in preventing unnecessary invasive procedures and helpful for the prediction and diagnosis of PCa and csPCa when combined with PSA and PSAD [70].…”

“…In our experience, among 272 patients with TSV, only seven had introduction through the ejaculatory duct, whereas most (127 patients) patients had transutricular fenestration. Especially in the situation of Type IV, the surgical path could not be established in other studies, 5 6 leading to failure of the procedure. In the present study, under real-time transrectal ultrasound guidance, the success rate of TSV was 96.8%, which is superior to similar previous studies.…”

“…However, TSV is a challenging procedure, especially for surgeons just beginning to use this procedure. As demonstrated by Chen et al ., 5 there are several issues preventing wide application of this procedure. First, detailed surgical procedures for TSV are currently nonstandardized.…”

“…The main methods of seminal vesicle and ejaculatory duct evaluation include TUSVE and seminal vesicle MRI. TUSVE, which has the advantages of using nonionising radiation and being fast, noninvasive/less invasive and inexpensive, has become the preferred evaluation method for EDO (Chen et al, 2018). However, it has the following two recognised disadvantages: (a) lower resolution makes it impossible to distinguish the position of the ejaculatory tube; (b) it is highly dependent on the operator's skill and description ability.…”
