Treatment With Acyclovir Does Not Affect Orogenital Ulcers in Behcçet's Syndrome: A Randomized Double-Blind Trial

Abstract: Acyclovir is a potent antiviral drug, of proven efficacy in the treatment of herpes simplex virus infection. Since this virus has been implicated in the aetiology of Behçet's syndrome, the effect of acyclovir on orogenital ulceration in this condition was determined in a randomized, double-blind, placebo-controlled, crossover trial. Eighteen of 22 patients entering the study completed the trial. Treatment with acyclovir failed to alleviate the frequency and severity of orogenital ulceration or other disease fe… Show more

“…Topical treatment with CS, topical anesthetics, antimicrobial mouthwashes, sucralfate [Keogan, 2009] and topical granulocyte colony-stimulating factor [Bacanli et al 2006] can be helpful in the management of mucocutanous lesions. Additionally, a number of antibiotics and antivirals, based on the possible role of herpes and Streptococcus sanguinis in the pathogenesis of the disease, have been used primarily for the treatment of mucocutaneous manifestations with variable responses [Calguneri et al 1996a[Calguneri et al , 1996bDavies et al 1988] and thus are not part of the routine care of patients with BD.…”

“…Innumerable attempts to demonstrate an infectious agent in the eye have proven fruitless. Additionally, a recently completed double masked randomized trial in which acylovir was used to treat the orogenital ulcers seen in this disease failed to show any efficacy [12]. However, an environmental role must be considered.…”

“…By use of DNA-DNA dot blotting and polymerase chain reaction (PCR) analysis, HSV DNA also was detected in leukocytes and saliva from patients with BD [8,9]. Treatment with acyclovir, which is of proven efficacy in the treatment of HSV infection, did not much alleviate the frequency or severity of orogenital ulceration or other clinical features of BD [10]. Lymphocyte dysfunction has been reported by several research groups.…”
