Tremor after long term lithium treatment; is it cortical myoclonus?

Abstract: Introduction Tremor is a common side effect of treatment with lithium. Its characteristics can vary and when less rhythmical, distinction from myoclonus can be difficult. Methods We identified 8 patients on long-term treatment with lithium that developed upper limb tremor. All patients were assessed clinically and electrophysiologically, with jerk-locked averaging (JLA) and cross-correlation (CC) analysis, and five of them underwent brain MRI examination including spect… Show more

“…Gluten sensitivity is a well-recognised cause of cerebellar ataxia with cortical myoclonus and in this instance could have been a synergistic factor leading to the development of cortical myoclonus [ 18 ]. We have described a similar phenomenon in a cohort of patients with long-term exposure to lithium who were also gluten sensitive and developed cortical myoclonus [ 29 ]. In addition, our patient with the IgA deficiency was found to have evidence of exaggerated physiological acoustic startle responses.…”

“…Risk factors have been identified with drug-induced movement disorders. Lithium-induced tremors are usually seen in the elderly, with toxic lithium levels, preexisting antidepressant usage, excessive caffeine intake, personal or family history of tremors, alcohol withdrawal, and anxiety [ 11 , 15 ]. Thus, clinicians need to be aware of the risk factors when starting a patient on lithium and monitor lithium levels.…”

“…2 Lithium an alkali metal discovered in 1817 was used in 19 th century for bipolar disorder and depression. 3,4 A permanent cerebellar syndrome was reported by the use of Lithium 5 and methylcobalamin promoted the normal histology and thickness of gray matter 6 .…”
