“…We excluded multiple gestations and breech presentations (n=332,719), identified using either the birth certificate checkboxes or International Classification of Diseases, 9th revision, clinical modification (ICD-9) codes in the discharge file. Records with previable gestational ages (< 23 weeks gestation [14, 15]) were also excluded (n=6,946).
