Trigeminal Neuralgia Caused by an Anomalous Posterior Inferior Cerebellar Artery from the Primitive Trigeminal Artery: Case Report

Abstract: A 61-year-old woman presented with typical trigeminal neuralgia (TN), caused by an aberrant posterior inferior cerebellar artery (PICA) associated with the primitive trigeminal artery (PTA). Magnetic resonance angiography and digital subtraction angiography clearly showed an anomalous artery directly originating from the PTA and coursing into the PICA territory at the cerebellum. During microvascular decompression (MVD), we confirmed and decompressed vascular compression of the trigeminal nerve by this anomalo… Show more

“…TN is an paroxysmal, lancinating and annoying facial pain syndrome occurring in the region innervated by the trigeminal nerve (CN V). The most common offending vessels are the SCA, AICA and vertebrobasilar trunk 133 , 134 . Very rarely, TN could result from the PICA associated with the PTA as mentioned before 31 .…”

“…Several reported cases of PTA-induced TN utilized cerebral angiography for verification of the PTA4,8,9). In our case, we intended to implement cerebral angiography for PTA identification; however, we used MRA only.…”

“…The vessel that most often causes TN is the superior cerebellar artery (SCA), other known offending vessels include the anterior inferior cerebellar artery (AICA) and the vertebrobasilar trunk and vein. Case reports have also rarely described TN caused by a primitive trigeminal artery (PTA)2,4,8,9). We introduce a case of TN caused by PTA that was successfully treated with surgery.…”

“…The posterior inferior cerebellar artery (PICA) usually arises from the intracranial portion of the vertebral artery. However, the PICA has numerous variations, and can emerge from the extracranial portion of the vertebral artery, 1) internal carotid artery, 2) primitive trigeminal artery, 3) primitive hypoglossal artery, 4) or posterior meningeal artery. 5) The authors report an extremely rare variation in which the PICA arose from the jugular branch of the ascending pharyngeal artery (APA).…”
