Background: Polycystic ovary syndrome (PCOS) is a prevalent hormonal disorder connected to ovulatory infertility and recent research indicates that different diets alter the risk of developing PCOS. Insulin resistance (IR) is one of the key pathologies which classifies PCOS. Understanding the etiology leading to IR in PCOS patients and the disorders associated with IR can allow for a therapeutic measure to be put in place to counteract the etiology and even reduce the risk of developing the disorder. Objective: The objective of this review is to identify the key factors that lead to IR so that diet and other lifestyle aspects can be used to counteract PCOS Symptoms.Design: A total of 679 studies were taken into consideration for this review, however, I performed a literature review of 75 primary papers. Data and findings from these studies were used to gain a better understanding of the dietary etiology of IR in PCOS, and this information can be used to describe a specific diet to counteract its etiology.Results: Many symptoms and disorders in the biological pathway of PCOS is associated with IR. Clinical studies have established that an increase in carbohydrates, low-density lipoproteins, and triglycerides is associated with IR in PCOS patients. Similar studies have found that a decrease in dietary proteins (especially branched chain amino acid intake), lactate, phosphatidylcholine, high-density lipoproteins, and long-chain fatty acids are also associated with PCOS patients. The drug metformin has also been associated with ameliorating many of the negative impacts imposed by IR in PCOS patients. Addressing these factors and changing diet and lifestyle (such as increased exercise) can help reduce the IR-related effects in PCOS patients.

Background Polycystic ovary syndrome (PCOS) is characterized by reproductive disorder and increased risk of metabolic syndrome. This study aimed to assess the metabolic parameters in the cord blood of neonate of mothers with obese PCOS and comparison with non-obese PCOS and controls. Methods This retrospective cohort study was conducted in Arash and Kamali Hospital in 2017–2018. The biochemical test was conducted on 78 neonates from obese PCOS mothers, 78 neonates from non-obese PCOS mothers, and 78 neonates from healthy mothers. Finally, cord blood lipid profile and insulin and blood sugar were determined by specific kits. Correlations between variables were compared with chi-square, Mann-Whitney’s U, Kruskal-Wallis H tests and regression model by SPSS 23 and P < 0.05 was considered significant. Results Triglycerides (TG) and high-density lipoprotein cholesterol (HDL) were higher in cord blood of newborn of obese PCOS women than non-obese PCOS and controls (P = 0.02, P < 0.001, respectively). Also, the mean insulin was higher in cord blood of neonate of non-obese PCOS women than in obese PCOS and controls (12.26 ± 12.79 vs. 11.11 ± 16.51 vs. 6.21 ± 10.66, P = 0.01). But in the study, there was no significant difference between the mean of umbilical cord low-density lipoprotein cholesterol (LDL), total cholesterol and blood sugar in three groups. The logistic regression model showed that metabolic parameters were related to PCOS. Conclusions In the present study, there was a significant difference between the mean of umbilical cord HDL, cholesterol, and the insulin level in the three groups. But, there was no significant association between the mean of blood sugar, LDL, and TG in the groups. The metabolic disorder in PCOS might affect cord blood lipid and insulin and adulthood health.

Nonalcoholic fatty liver disease (NAFLD) describes a spectrum of liver damage due to excessive hepatic lipid accumulation. Recent research has demonstrated a high prevalence of NAFLD in women with polycystic ovary syndrome (PCOS). Strong associations independent of BMI have been found between high androgen levels characteristic of PCOS, as well as insulin resistance, and the presence of NAFLD in these women, suggesting that these factors contribute to liver injury more significantly than obesity. Current studies indicate the occurrence of NAFLD in normal weight women with PCOS in addition to the commonly researched women who are overweight and obese. While the majority of studies address NAFLD in adult, premenopausal women (ages 25-40 years), the occurrence of NAFLD in young and adolescent women has gone largely unaddressed. Overall, research in this field lacks diversity; a majority of studies either focus on populations of white women or are missing demographic information entirely. Future studies should include larger, more racially and ethnically inclusive populations and particular attention should be paid to how excess androgens and insulin resistance contribute to the increased risk of NAFLD seen in women with PCOS of varying weights, ages, and ethnicities. Here, we review NAFLD in women with PCOS with subsections focused on the impact of hyperandrogenism, BMI, insulin resistance and age. Most notably, we present the most up-to-date racially and ethnically diverse worldwide prevalence of NAFLD in women with PCOS compared to women without PCOS (51.56% vs. 29.64%, p<0.001, respectively).
