TTF-1/p63-Positive Poorly Differentiated NSCLC: A Histogenetic Hypothesis from the Basal Reserve Cell of the Terminal Respiratory Unit

Abstract: TTF-1 is expressed in the alveolar epithelium and in the basal cells of distal terminal bronchioles. It is considered the most sensitive and specific marker to define the adenocarcinoma arising from the terminal respiratory unit (TRU). TTF-1, CK7, CK5/6, p63 and p40 are useful for typifying the majority of non-small-cell lung cancers, with TTF and CK7 being typically expressed in adenocarcinomas and the latter three being expressed in squamous cell carcinoma. As tumors with coexpression of both TTF-1 and p63 i… Show more

“…We further validated the finding by both multiplex fluorescent IHC staining and RNA in situ hybridization. Although three recent papers reported three unusual cases of NSCLC with co-expression of TTF1 (NKX2-1) and p40 (TP63) in the same cells by IHC staining [ 51 54 ], they failed to observe the molecular heterogeneity present in an individual tumor. Our single-cell whole-transcriptional analysis of NSCLC is a powerful tool that provides molecular heterogeneity and cellular diversity in individual tumors and gives great promise in a more accurate classification of NSCLC.…”

“…These characteristics are present at the invasive site of the cancer, which appears as a non-lepidic region in lung cancer [ 9 ]. There are two types of non-lepidic regions (their presence is a surrogate sign of the invasive site of lung cancer) [ 23 ]: pure solid lesions and part-solid lesions with lepidic growth, where the malignancy potential is lower than that of a pure solid lesion. Furthermore, in the microenvironment of part-solid tumors, there is a lower chance of detecting hypoxia-induced factors (demethylation inducing), CAFs and TAMs [ 24 ], which are responsible for CTC clusters [ 25 ].…”

“…It has been broadly acknowledged that CK (7) positive refers to the epithelial-original cancer especially the lung tissues. [ 20 , 21 ] The National Comprehensive Cancer Network and Pan-Asian European Society for Medical Oncology guidelines of NSCLC strongly recommended to ascertain LUAD by TTF-1 and Napsin A. [ 22 , 23 ] Moreover, EGFR 19DEL has been proved 1 of the most common mutated forms in LUAD.…”
