Two Cases of Spontaneous Filtering Blebs, One Idiopathic and One Associated With Terrien Marginal Degeneration

Abstract: This is a rare case of an idiopathic filtering bleb occurring in a healthy patient with no precipitating cause, other ocular abnormalities, or physical malformations, and the only reported case of bilateral sequential filtering blebs associated with an atypical presentation of Terrien marginal degeneration.

“…Our patient had iridocorneal adhesion only in the LE and did not have anterior embryotoxon in both the eyes. Spontaneous filtering bleb formation due to corneal pathologies such as Terrien and pellucid marginal degeneration have been reported by Soong et al .,[ 2 ] Munro et al .,[ 3 ] and Toriyama et al . [ 8 ] Mechanism proposed was the extreme distortion and stretching of the peripheral cornea resulting in rupture of the Descemet's membrane and bleb formation.…”

“…They are reported in association with ocular abnormalities like Axenfeld syndrome, Terrien's marginal degeneration, microspherophakia, ectopia lentis, scleritis, and pellucid marginal corneal degeneration. 2 , 3 , 4 , 5 , 6 , 7 , 8 There are reports of spontaneous filtering blebs in systemic conditions like scleroderma and Traboulsi syndrome. 9 , 10 , 11 , 12 In this study, we present a case with mild craniofacial dysmorphism, microspherophakia, and bilateral spontaneous filtering blebs.…”

“…Spontaneous filtering blebs are an uncommon entity, the appearance of which may be favored in patients with previous history of penetrating trauma 1 or penetrating ocular procedures, such as cataract surgery or scleral fixated intraocular lens (IOL) implantation. 2 They have also been described in cases without history of trauma or surgery but with ocular thinning abnormalities, such as Terrien’s marginal degeneration, 3 pellucid marginal degeneration, 4 Axenfeld syndrome, 5 scleritis, 6 microspherophakia, 7 systemic disorders like scleroderma, 8 and craniofacial dysmorphism, 9 or other connective tissue diseases with thin sclerae, such as high myopia. However, in some cases, the etiology cannot be determined.…”
