Two‐Dimensional Speckle Tracking Echocardiography in Heart Transplant Patients: Two‐Year Follow‐Up of Right and Left Ventricular Function

Abstract: In this series of HT recipients with uneventful postoperative course, LV and RV GLS values were significantly reduced early after HT and improved progressively until their complete normalization two and 1 year after HT, respectively. This is the first study to show a full recovery of LV and RV deformation parameters and offers "normal" strain values that, if confirmed in larger studies, could be useful for monitoring the evolution of HT recipients.

“…According to the left heart, the biplane LVEF was preserved after HTx, but the LS was significantly reduced at the follow‐ups. Similar research outcomes were evaluated by other studies 5,6,8,12–15 . In addition, we found the values including the global and segmental strain after 5 years were slightly higher than after 1 year, but 1‐, 5‐year follow‐ups were significantly increased than 1‐, 3‐month follow‐ups.…”

“…In addition, we found the values including the global and segmental strain after 5 years were slightly higher than after 1 year, but 1‐, 5‐year follow‐ups were significantly increased than 1‐, 3‐month follow‐ups. Moñivas et al 8 revealed that the global strain was reduced in the early stage after transplantation, but it was improved progressively until its complete normalization 2 years after HTx. The first study that described follow‐up deformation parameters in HTx patients indicated that patients with a normal EF showed deterioration of LS values at a 3‐year follow‐up after the initial assessment.…”

“…Furthermore, Moñivas Palomero et al [49] demonstrated that RV strain could be useful for monitoring the evolution of heart transplant recipients. They found that RV strain was significantly reduced early after heart transplant and improved progressively, reaching normal values one year after surgery.…”

“…Ez a klinikai kép egyéb szívsebészeti beavatkozásoknál is jelen van, és feltehetően multifaktoriális eredetű [42]. A jobb kamrai longitudinális funkció csökkenése megtartott kamrafunkció mellett speckle-tracking analízissel is észlelhető [43]. Mindezek az eredmények felhívják a figyelmet arra a kevéssé feltárt kérdéskörre, hogy a komplex működésű jobb kamra kontrakciós mintázata egyes állapotokban minőségileg és nem mennyiségileg módosulhat, illetve ennélfogva a TAPSE, mint rutin funkcionális paraméter szívsebészeti beavatkozásokat követően, így szívtransz-plantáció esetében sem ideális marker (5. ábra).…”
