Two patients with spontaneous transomental hernia treated with laparoscopic surgery: a review

Abstract: Here, we report two patients with transomental hernia who were successfully treated with laparoscopic surgery. The first patient was a 58-year-old female who presented to our hospital with abdominal pain and vomiting; she had no history of abdominal surgery. Enhanced computed tomography revealed strangulation ileus due to an internal hernia. The second patient was a 36-year-old male who presented to our hospital with abdominal pain and no history of abdominal surgery. Enhanced computed tomography indicated tra… Show more

“…In general, the preoperative diagnosis of an internal hernia itself is not challenging [5]. Characteristic CT findings and the patient's history allowed us to diagnose the internal hernia in this case; however, we could not identify the specific pathogenesis.…”

“…Transomental hernias are particularly rare, constituting only 1% to 4% of all internal hernias; only 24 cases have been reported in the English-language literature [5]. The pathogenesis of a transomental hernia involves the development of a congenital defect of the omentum and omental fragility caused by emaciation or atrophy of the omentum secondary to aging, oral steroids, trauma, surgery, or inflammation [3,4].…”

“…A transomental hernia is a rare cause of Not only in virgin cases small bowel obstruction that is occasionally diagnosed during surgery [1][2][3][4][5][6]. Preoperative diagnosis of transomental hernia is difficult, but abdominal CT can help to detect strangulation ileus resulting from transomental hernia as in these two cases [1][2][3][4]. A transomental hernia shows finding of small bowel obstruction.…”

“…Preoperative diagnosis is difficult because of a lack of specific clinical findings. Prompt laparoscopic surgery is effective for treating internal hernias and spontaneous transomental hernias without evidence of necrosis or perforation of the hernial contents [3,4]. We report consecutive two cases of transomental hernia through the greater omentum without any history of abdominal surgery or trauma.…”
