Ultrafast Toluidine Blue Staining for Rapid On-Site Evaluation of Cytological Smears

Abstract: Rapid on-site evaluation (ROSE) is one of cytopathology’s “unique selling propositions.” The quality, speed, and ease of handling of the staining used is a critical factor for the efficacy of the ROSE procedure. Here, we describe a modification of rapid toluidine blue staining that can be performed within 25 s, provides excellent nuclear morphology, and is compatible with subsequent Papanicolaou staining of the slides. Furthermore, exposure to hazardous chemicals is minimized, as no organic solvents other than… Show more

“…Traditional methods like Papanicolaou and Giemsa stains, despite their diagnostic clarity and established utility, pose challenges in terms of time and resource requirements [ 9 , 12 , 13 , 15 ]. This has catalyzed the exploration of alternative staining methods, as evidenced by the research conducted by Amannagi et al [ 16 ] and Hewer et al [ 17 ] and our own previous investigation [ 12 ].…”

“…Their findings underscore the importance of rapid staining techniques, highlighting how such methods can facilitate prompt preliminary assessments without compromising diagnostic accuracy. Similarly, Hewer et al [ 17 ] also investigated toluidine blue in thyroid FNAB, emphasizing its efficiency in cytology reporting. However, similar to Amannagi’s study [ 16 ], Hewer et al [ 17 ] concentrated primarily on sample adequacy rather than a comprehensive cytological evaluation.…”

“…There are some factors that make a rapid stain widely accepted for use. A rapid stain should provide an easy and successful fixation, or preferably, no requirement of fixation at all, cellular transparency, an easy and user-friendly staining process, and low cost [4,5]. For the most common rapid staining method, DQ, the entirely air-dried smears are "must-have" to provide a successful staining.…”

“…A good and correct adequacy assessment affects the success of a final report in cytopathology. There are a few stains available in the market for ROSE, namely, Diff-Quik (DQ), Toluidine blue, and ultrafast Papanicolaou stains [4,5]. Of those, the most common in use is DQ [6].…”

“…The excess dye was removed and the cells were washed thrice with PBS (2 min/step). The cells were imaged using an optical microscope (XDS-1A) and an inverted microscope (Olympus, IX71) ( Hewer and Schmitt, 2020 ).…”
