Ultrasound-Guided Peripheral Nerve Stimulation for Shoulder Pain: Anatomic Review and Assessment of the Current Clinical Evidence

Abstract: Background: The shoulder region is a common area for pain. The shoulder has the largest range of motion and the most complex mechanical anatomy. The shoulder girdle and related tendons allow for numerous painful disorders to occur. Also, given the overall use of the shoulder, arthritic deformities are all too common. Finally, pain from more complex states such as poststroke shoulder pain and status post total shoulder arthroplasty pain have always been a difficult diagnosis to treat with effectiveness. The inn… Show more

“…The quadrangular space is one of three axillary spaces and is made up of the teres minor superiorly, teres major inferiorly, long head of the triceps brachii medially, and laterally by the neck of the humerus containing the axillary nerve and the posterior humoral circumflex artery (PHCA). 102,103 With the patient in the lateral or prone position, the ultrasound probe is placed posteriorly and vertically, in-plane with the humeral shaft. The probe is moved superiorly toward the surgical neck of the humerus.…”

“…Ultrasound-guided lead placement has been studied for the shoulder, which provides an alternative to open placement for deeper nerve structures. The suprascapular and axillary nerves can be targeted from any location throughout their courses, although the suprascapular notch and the posterior humerus, respectively, are the most common locations picked [ 30 , 32 , 33 ]. For either nerve, a needle can be advanced under ultrasound guidance until it is just past the nerve.…”

“…45 Interestingly, the data are most robust for successful PNS therapy for poststroke shoulder pain. 35,48 Although the pathologies and etiologies described previously are undoubtedly different from post-TSA/RSA pain, these successful cases may provide a framework to consider PNS for post-TSA/RSA pain. Hopefully, this can be further explored in future research and clinical reports.…”
