Understanding the Host in the Management of Pneumonia. An Official American Thoracic Society Workshop Report

Abstract: Pneumonia causes a significant burden of disease worldwide. Although all populations are at risk of pneumonia, those at extremes of age and those with immunosuppressive disorders, underlying respiratory disease, and critical illness are particularly vulnerable. Although clinical practice guidelines addressing the management and treatment of pneumonia exist, few of the supporting studies focus on the crucial contributions of the host in pneumonia pathogenesis and recovery. Such essential considerations include … Show more

“…In the lung, this process results in severe pneumonia, a leading cause of human death even before the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic ( 3 , 4 ). The outcome of pneumonia is determined by a balance between immune resistance and tissue resilience ( 1 , 5 ). Immune resistance is the host’s ability to eradicate pathogens, whereas tissue resilience is the ability of the lung to withstand the deleterious consequences of infection and inflammation ( 1 , 5 ).…”

“…In severe pneumonia, endpoints such as death or extubation failure are easily ascertained, while other endpoints like clinical cure, microbiologic resolution, improvement in oxygenation and safety are not standardized and subject to bias in interpretation. 1 In studies with complex endpoints of interest, particularly events that require some analysis of clinical information, study protocols use adjudication committees to provide consistent, unbiased determination of the presence or absence of a diagnosis or an event. 24 Adjudication committees are established components of large cohort studies and randomized clinical trials in cardiovascular disease, oncology, interstitial lung disease and sepsis.…”
