Unique Spontaneous Coronary Artery Dissection in an Elderly Woman

Abstract: Spontaneous coronary artery dissection is a rare condition, typically occurring in younger women. A 75-year-old woman with this condition, 17 years older than previously reported, is described, including her unusual presentation with angina rather than infarct. The 112 previously reported cases are summarized and treatment options discussed.

“…Our patient is the third reported case of SCAD in someone aged > 70 years, and the first patient to have dissections of the LAD, the first major diagonal and the RCA at the same time. The other two cases in elderly patients were reported by Laslett et al 8 and Sakamota et al 9 In conclusion, repetitive mechanical stress and heavy lifting by a diabetic patient who has no indication of atherosclerosis in the coronary arteries may cause multiple SCADs. …”

“…It is associated with pregnancy during the third trimester and 3 months postpartum (21.4%). [2][3][4][5][6][7][8] The pattern and severity of presentation are variable and related to the extent of dissection, its rate of development, and the vessel involved. 3,9 In women with SCAD there is a predilection for the left coronary system (84%), whereas in men, the right coronary artery is usually affected (67%).…”
