Update on the prevention, diagnosis and management of cytomegalovirus infection during pregnancy

Abstract: Human cytomegalovirus (CMV) is the leading cause of congenital infection, with morbidity and mortality at birth and sequelae. Each year approximately 1-7% (Rev Med Virol 2010; 20: 311) of pregnant women acquire a primary CMV infection. Of these, about 30-40% transmit infection to their fetuses. The risk of serious fetal injury is greatest when maternal infection develops in the first trimester or early in the second trimester. Between 10 and 15% of congenitally infected infants are acutely symptomatic at birth… Show more

“…Whether evaluating CMV viral loads in amniotic fluid predicts disease severity in newborns remains controversial. Some authors suggest that viral loads Ͼ10 5 genome equivalents (GEq)/ml are highly indicative of symptomatic disease, whereas values Ͻ10 3 GEq/ml exclude severe cCMV forms (19,39,40). Opposing results regarding the association between symptomatic disease and viral loads in amniotic fluid have been reported.…”

“…CMV infection is generally asymptomatic in healthy, immunocompetent individuals but can cause severe diseases in immunodeficient patients. It is the major viral cause for congenital birth defects such as mental retardation and deafness in the United States (39). HCMV can also cause disease in solid organ and bone marrow transplant recipients, where the virus can reactivate from either the transplanted organ or the CMV-positive recipient and lead to disease or graft rejection (33).…”

“…This test shows the strength with which the antibody binds to the antigen (25) . At the beginning of infection, the produced IgG shows a low avidity for the virus that increases within weeks after exposure to the antigen.…”

“…Although there is no vaccine available to prevent CMV infection, simple measures (such as hand washing after contact with saliva or urine, and avoiding to share glasses and cutlery amongst others) (25,28) are proven to be effective in order to minimize virus transmission.…”
