Upper abdominal lymph nodes: criteria for normal size determined with CT.

Abstract: Reports of the upper limits of normal for lymph node size at abdominal computed tomography have varied from 6 to 20 mm. Establishment of an upper limit for node size by specific location, analogous to that which has been reported for mediastinal lymph nodes, was sought. Short-axis diameters of the lymph nodes were measured in 130 patients who were not likely to have enlarged abdominal lymph nodes. Seven locations were defined, and the largest nodal measurement for each was recorded. Histographic analysis and n… Show more

“…13 Regional lymph nodes were considered to be involved when the short-axis diameter was 96 mm for perigastric lymph nodes and 98 mm for extraperigastric lymph nodes. 14,15 For N staging, the new TNM 5 and Japanese classifications 6 were independently used. In the new TNM classification, N stage was determined by the number of metastatic lymph nodes from a minimum yield of 15 total lymph nodes (N1, 1Y6 involved nodes; N2, 7Y15 involved nodes; and N3, 915 nodes).…”

“…The wall thickness measured on CT was divided into four groups according to the depth of invasion (mucosa, submucosa, muscle, and beyond serosa) on the histopathologic specimen, and the mean value of the wall thickness of each group was calculated. The presence of lymphadenopathy was determined using the size criteria established by Dorfman et al [20]. Accordingly, short-axis nodal diameters greater than 6 mm in the retrocrural space, 7 mm in the porta hepatis, 8 mm in the gastrohepatic ligament region, and 10 mm in all other sites were considered abnormal.…”
