Upper gastrointestinal bleeding in neck of femur fracture patients: a single tertiary centre experience

Abstract: This study aimed to identify the incidence of perioperative acute upper gastrointestinal bleeding (UGIB) in our hip fracture patients; to evaluate the characteristics, management and clinical outcomes of these patients; and to explore risk factors and protective factors. Of the 1691 consecutive patients admitted for surgical management of hip fractures, 11 (0.65%) had UGIB and a further four patients for each case were selected as controls for evaluation of risk factors and protective factors. Pre-existing pep… Show more

“…In meta-analyses of studies that examined medication adherence and outcomes in type 2 diabetes (largely in middle-aged subjects), better adherence has been associated both with a lower rate of hospitalisation and lower all-cause mortality. 15,18 Our study's limitations include that it is retrospective and observational, and that we have not accounted for partial adherence in subjects taking more than one medication. It is possible that adherence to insulin treatment might be poorer than to oral medication, but we did not analyse this.…”

“…The reported incidence of perioperative acute upper gastrointestinal bleeding varies widely in the literature but nowadays seems low [75]. Pre-existing peptic ulcer disease and non-steroidal anti-inflammatory drugs are known risk factors while post-operative use of aspirin is not likely to be a strong risk factor.…”

“… 5 The mortality rate in hip fractures in the elderly is approximately 10% over the first month and 33% within the first year. 1 , 4 , 6 , 7 Peri-operative complications including gastrointestinal bleeding are the main determinants of outcomes and the leading cause of morbidity and mortality in elderly patients with hip fractures. 3 , 6 8 …”
