Background: Opportunistic infections and malignancies cause lymphadenopathy in HIV-infected patients. The use and accuracy of fine needle aspiration cytology in diagnosing of cervical lymphadenopathy among HIV-infected patients is not well studied in Uganda. Objective: The aim of this study was to determine the diagnostic accuracy of fine needle aspiration cytology in providing a diagnosis of cervical lymphadenopathy among HIV-infected patients in Uganda. Methods: We consecutively recruited adult HIV-infected patients with cervical lymphadenopathy admitted to Mulago Hospital medical wards. Clinical examination, fine needle aspiration and lymph node biopsy were performed. We estimated the sensitivity, specificity; negative and positive predictive values using histology as the gold standard. BACKGROUNDPrevalence of Granulomatous lymphadenitis in human immune deficiency virus infection is increasing drastically in developing countries, particularly in India. In the present study, an attempt has been made to find an association of Granulomatous lymphadenitis in human immune deficiency virus infection in humans.The aim of this study is to correlate the FNAC finding with clinical, radiological and morphological patterns like correlation with findings and the CD4 count, relation of Mantoux to lymph nodes and relation with x-ray findings. MATERIALS AND METHODSA descriptive study done in 100 adult patients, aimed to study the prevalence of tuberculous lymphadenitis by cytological patterns in lymph nodes of HIV sero-positive patients by collecting and studying material through FNAC and clinical importance by Mantoux and x-ray findings. Patients attending Government General Hospital, ACSR GMC, Nellore who was not on ATT or ART were chosen for the study. RESULTSVarious lesions were observed in this study. Granulomatous lymphadenitis was observed in 74 cases, reactive lymphadenitis was seen in 22 cases and acute suppurative lymphadenitis was seen in 3 cases. One case has shown secondary deposits of squamous cell carcinoma. CONCLUSIONFNAC is a relatively inexpensive initial investigative technique important for assessing the cause of lymphadenopathy in HIV patients; allows a rapid diagnosis obviating the need for surgery and enabling swift treatment to be undertaken where necessary. Universal safety precautions must be strictly followed. The method is accurate, cost effective and acceptable to patients. TB is the major opportunistic infection, which could be detected in both symptomatic and asymptomatic individuals and even in tiny nodes. There was strong association with Mantoux and x-ray for diagnosing Granulomatous lesions. KEYWORDS

Las adenopatías son un hallazgo frecuente en pacientes con infección por el virus de la inmunodeficiencia humana (HIV). El diagnóstico diferencial incluye infecciones, neoplasias malignas o cambios reactivos. No existen datos sobre su etiología en Suramérica. Objetivo. Describir la etiología de la histopatología ganglionar en pacientes con HIV del Hospital La María de Medellín. Materiales y métodos. Se evaluaron de manera retrospectiva historias clínicas de pacientes con HIV y linfadenopatías, a quienes se les había hecho biopsia por escisión de ganglio linfático entre junio de 2009 y octubre de 2011. Se registraron los datos de los resultados de las biopsias en cuanto al estado de inmunosupresión, el uso de tratamiento antirretroviral y el diagnóstico final de los pacientes. Resultados. Se evaluaron 120 historias clínicas, con diagnósticos distribuidos así: 58 % de origen infeccioso, 32,5 % con cambios reactivos, 6,6 % de origen neoplásico y 2,5 % normales. El diagnóstico más frecuente fue la tuberculosis (48,3 %). En 14,1 % de los pacientes, la biopsia de ganglio linfático sirvió para detectar otra infección oportunista además de las encontradas en otros órganos. Conclusión. La biopsia de ganglio linfático en pacientes con HIV es una herramienta útil que permite el diagnóstico de enfermedades infecciosas y neoplásicas graves, y que debería hacerse de rutina en todos los pacientes con adenopatías.
