Usefulness of conventional transbronchial needle aspiration in the diagnosis, staging and molecular characterization of pulmonary neoplasias by thin-prep based cytology: experience of a single oncological institute

Abstract: The c-TBNA proved to be an efficient method for the diagnosis/staging of lung neoplasms and for the diagnosis of mediastinal lymphadenopathy. Endoscopist's skill and technical development, associated to thin-prep cytology and to a rapid on site examination (ROSE), were able to provide by c-TBNA a high diagnostic yield and molecular predictive data in advanced lung carcinomas.

“…At the preoperative stage, lymph nodes were considered negative when the CT scan showed a short-axis < 1 cm and/or when the standardized uptake value was <3 in the PET scan, in accordance with the guidelines of nuclear medicine physicians. In the event of nodes > 1 cm and SUV in PET scan < 3, an endobronchial ultrasound transbronchial fine needle aspiration (EBUS-TBNA), an endoscopic ultrasound fine needle aspiration (EUS-FNA), mediastinoscopy, or VATS was performed to exclude malignancy ( 12 ). Before the operations were carried out, all patients had signed an informed consent to undergo a lobectomy.…”

“…It can remove the influence of blood and mucous fluid, making the background of smear clean and the cell structures more clear. [7,8] but is seldom used in TBNA samples. Gang Hou et al [9] found that the sensitivity of TBNA samples was 64.7% (65/102) by the traditional direct smear method, while the sensitivity by LBC was 59.8%…”
