Using Telecommunication Technology to Manage Children with Diabetes: The Computer-Linked Outpatient Clinic (CLOC) Study

Abstract: The purpose of this study was to evaluate the efficacy of using a telecommunication system to assist in the outpatient management of pediatric patients with insulin-dependent diabetes. Metabolic control, patients' psychosocial status, family functioning, perceived quality of life, patterns of parental/child responsibility for daily diabetes maintenance, and nursing time-on-task were evaluated. One hundred six pediatric patients (mean age = 13.3 years) were randomly assigned to an experimental or control outpat… Show more

“…In 14 (20,21,23,28,29,31,39,44,(51)(52)(53)58,59,64) of these, the guidelines were locally developed, in 11 studies (22,24,26,27,32,35,36,47,55,57,60) they were based on national guidelines, and in 2 studies (46,54) the source of the guidelines was not specified.…”

“…In 18 studies (21,(27)(28)(29)33,34,36,37,40,41,47,51,52,56,60,61,63,66), the outcomes were all reliably assessed (outcomes obtained from an automated system or a reported agreement between two raters Ͼ90% or Ն 0.8). Furthermore, 15 studies (20,22,26,32,35,39,44,46,50,53,54,57,59,62,64) included objective laboratory assessment of glycemic control.…”

“…Our study shares the limitations of previous studies, which include enrolling a small sample, following them for a brief period (maximum follow-up was 12 months in two studies [26,29], 6 months in four others, and 3 months in another one [24]), and losing about 10% of patients to follow-up. The strongest inference about the true magnitude of telecare effect on glycemic control comes from our meta-analysis.…”

“…Systematic review and meta-analysis of the literature Two reviewers working independently considered six trials published in seven manuscripts (22)(23)(24)(25)(26)(27)(28) eligible for review, with perfect interobserver agreement (Table 2). One additional eligible study was presented as a poster at the 63rd Scientific Sessions of the American Diabetes Association in June 2003 (29).…”

“…Por lo tanto, aunque el diseño, desarrollo e implementación de los sistemas WSN/WBAN representa una tarea difícil con varias cuestiones de una elevada dificultad técnica, los beneficios de su utilización para la prevención, diagnóstico precoz y gestión de las enfermedades son múltiples y ampliamente documentados (141,(154)(155)(156)(157)(158)(159)(160) .…”
